You need to consider the armors natural color
If the color of your armor (from the crafter) is brown then you have to start your mix with brown as your base color. Since dye remover is actually a dye which will dye your armor its original color then the dye remover is actually a brown dye.
So consider that the starting color or base color should be the dye remover. If you can get white starting with the dye remover then you did it. Chances are is not likely. Since White is a lack of color it will be impossible to mix a white dye from other colors. You may get close by using the closest color to white which is silver to the dye remover (base color) but not likely.
If you base color is a dark color your best bet is to go with yellow=silver+silver+silver.
Has anyone tried to trade this mix with another user? Can you add more dyes a vial of dye which has 4 mixes to it already if you get it from someone else?

I see you have 15k plate on your warrior in you Avatar, I used silver+silver+silver+dye remover, and IMO it looks very good on 15k plate.

silver dye+dye remover+silver dye gave me a pretty good result. Dont have any pics to show you, but it's kinda of a metallic off white.

White dye combo : Yellow+silver+silver+silver on a Dragon armor will give you a black gray. Worked very well and i liked it better than black.

Here is the formula for that pink/magenta of mine, now go check that chart and then try it ingame on some armor. Blue+Orange.

i wish in factions they would just make a white dye that we could buy at the traders. i hate seeing ppl in town saying oh i have the best white dye formula it works great come buy it now for the cheap price of only 1k per bottle. the part they dont tell you is they wont tell you the formula and to top it off most of the time it doesnt even work on all armor so it ends up looking like major crap on alot of armor. they just need to make a white dye and sell it at the dye trader.

Something that has not been addressed is your graphics card and guildwars
settings affect the colour imensly, if i turn my settings down to the lowest
silver looks "grey" to "white" but if i have it set up all at it's highest i get a
nice bright silver, .
Before you all go thinking you dont have the right colour mixed or it simple
doesnt work, change your graphics settings and see the results for yourself...

Not sure if this has been mentioned, but having read last 2 pages it doesnt look like it.
This is pure speculation nd I just have one example to prove it but i've found adding dye remover into a concoction basically results in the base armour colour effectively acting as the dye remover portion of the overall mix.
For example if you have monks with their light tan base armour, yellow+silver+tan would give u a whitish colour.
However, the thing that made me come ot this conclusion was using the Silver+Dye Remover+Silver on a 15k ascalon warrior helm which has a base colour of gold. Applying the mix resulting in a visibly lighter gold. I believe all the S+DR+S mix does is to lighten the natural colour of your armour hence the mixed results everyone is getting.
